Eldercare Assessment
Conduct comprehensive medical, functional, cognitive, psycho-social, financial and safety assessments
Determine the level of care needed
Customized Plan of Action
Develop an individualized plan of care with the input of the client, family, and community professionals to address unmet needs
Evaluate availability, cost and quality of community resources and services
Care Coordination and Advocacy
Assist client/family to hire in-home caregivers and monitor services rendered
Accompany client on medical appointments, as necessary, to insure communication is timely, complete and accurate
Coordinate and monitor medical instructions with multiple physicians and specialists to ensure follow-through
Provide medication management
Provide friendly visits and telephone reminders/reassurances, as needed
Monitor whether all care and services delivered are reliable, compassionate and high quality.
Offer crises management
Service Referrals
Collaborate with client’s attorneys, financial advisors, insurance agents, community care providers, and involved government agencies
Identify and monitor maintenance providers regarding home repairs and modifications, auto maintenance, yard care, house cleaning
Financial Management
Assist with daily money management, including timely bill payments, bank statement reconciliation, health insurance claims
Assist with written inventory of important documents for applications and important legal paperwork
Complete paperwork necessary for federal/state benefits
Life Transitions
Insure safe transfer to/from hospital and rehabilitation facilities
Oversee relocations from home to alternative living arrangements
Discuss with client and family end-of-life issues including advance directives and hospice care
Client/Family Consultation
Facilitate family meetings to help with complex care decisions
Act as an impartial party when mediating family differences
Provide short-term respite supervision for the elderly during caregiver business trips or family vacations
Act as liaison and advocate for family members living out of state or in the next county to ensure their loved ones are safe and well
Counsel individuals with aging and caregiving issues
